Description: Minnesota Help Me Connect is an online navigator under development that is designed to help providers connect pregnant individuals and families with young children (birth – 8 years old) with services available in their local communities. The navigator was developed based on community-level recommendations from a central intake work group, navigator focus groups, early childhood provider interviews, and tribal consultation. This system will support local providers who have trusted relationships with young families to locate available services in the community and make referrals. The Help Me Connect (HMC) Planning Team from the MN Departments of Education, Health, and Human Services invites you to learn about the history, development, and upcoming launch of the Help Me Connect navigator. The planning team recently presented at the National Help Me Grow virtual forum to over 350 attendees from around the country. The team will give a modified version of that presentation via WebEx on the dates/times listed below. The presentation will include the same content on both dates. All attendees will be in listen mode and there will be electronic Q&A opportunity at the end of the presentation. There is no advanced registration for these sessions, just click and join on the date/time listed below. Please note: there is a cap of 300 for each presentation. Additional training and informational sessions will be held throughout this fall and winter.
